Chartered campus organizations that need verification of the enrollment status, grade point average, or other information of members of their organization may use the Organizational Information/Verification Request Form to file their request with the Registrar's Office.

Each member of the organization for whom information is requested must provide their student ID number and signature in order for the Registrar's Office to process the request.  Any misuse or disclosure of the information provided, or any misrepresentation or forgery of the names or signatures of the members of the organization, may result in disciplinary action under the Student Conduct Code.

Requests generally take up to two-three days to process, although a longer processing time may be required.
